Service Description

Stop losing revenue to preventable denials. This service provides comprehensive, portal-based insurance verification completed 3 to 5 days prior to the Date of Service (DOS). Deliverables: Verification of active benefits and exact patient responsibility (co-pays, deductibles, co-insurance). Identification of Primary and Secondary payers and Coordination of Benefits (COB). Direct documentation of all specific coverage details into your EMR system. Service Guarantee: I guarantee a standard of 100% accuracy on all portal checks. In the event of an error, the file will be re-verified at no additional cost. Terms of Agreement & Operations: Client Obligations: The Client agrees to provide active, secure access to all necessary EMR systems, clearinghouses, and payer portals required to perform the service. The Client must provide accurate patient demographic data and requested verification lists at least 3 to 5 days prior to the DOS. Contractor Status: I operate strictly as an Independent Contractor. This service does not create an employer-employee relationship. I am responsible for providing my own equipment and managing my local tax obligations. Compensation & Payments: The fee is $2.00 per portal check. Any verification requested with less than 3 days' notice is classified as a "Rush Request" and incurs a 50% surcharge. Invoices are generated based on the volume of completed verifications and are due per the agreed invoicing schedule. Data Security & Privacy: Strict compliance with HIPAA regulations is maintained at all times. All Protected Health Information (PHI) accessed during verifications is handled with maximum security and confidentiality, and is never stored on unauthorized local drives. Non-Solicitation: During the term of this service and for a standard period thereafter, neither party shall directly or indirectly solicit or recruit the other party’s employees, contractors, or clients. Term, Selection & Termination: This service agreement commences upon booking and continues until terminated by either party. Either the Client or the Contractor may terminate this ongoing service arrangement by providing written notice as outlined in the formal onboarding agreement.
